What Is PMOS (Formerly PCOS)?

Quick answer: PMOS (Polyendocrine Metabolic Ovarian Syndrome) is the updated name for the hormonal condition long known as PCOS (Polycystic Ovary Syndrome). It happens when the body produces higher than normal levels of androgens, which can disrupt ovulation and lead to irregular periods, acne, excess hair growth, painful periods, and difficulty conceiving. PMOS affects roughly 1 in 10 women.1,2 Despite its high prevalence, many cases go undiagnosed for years.3,4 The condition is also closely linked to insulin resistance, making it as much a metabolic issue as a reproductive one.

Why Did PCOS Change to PMOS?

In 2026, a global coalition of researcher, clinician and patient organizations formally retired the name Polycystic Ovary Syndrome in favor of Polyendocrine Metabolic Ovarian Syndrome. The change is intended to shift the focus on the metabolic dimension of the condition, rather than the presence of cysts (which aren’t even necessary to diagnose PMOS).

Here's what every woman should know about PMOS: what it is, how to recognize it, why insulin resistance is part of the conversation, and where to find care that treats the whole picture.

What Is PMOS (Formerly Known as PCOS)?

PMOS is one of the most common hormonal disorders affecting women, caused by elevated levels of androgens. Androgens are male hormones that every woman produces (like testosterone), just usually in smaller amounts. When androgen levels rise too high, they interfere with ovulation, which throws off the menstrual cycle and triggers a cascade of other symptoms.1,2

The name change reflects a shift in how the medical community understands this condition. "Polycystic" suggested the issue was mainly about ovarian cysts, but many women with PMOS don't have true cysts at all. The new name centers the bigger picture: PMOS is a whole-body endocrine and metabolic condition, not just a reproductive one. For patients, that reframing matters as it opens the door to more complete screening instead of symptoms being dismissed as unrelated.

What Are the Symptoms of PMOS?

PMOS can show up differently from person to person, but some common symptoms include:5

  • Irregular or unpredictable menstrual periods
  • Weight gain or difficulty with weight loss
  • Thinning hair
  • Ovarian cysts
  • Excess facial and body hair (hirsutism)
  • Persistent or hormonal acne (especially along the jaw line)

You don't need every symptom on this list to have PMOS, and symptoms can range from mild to significantly disruptive. If several of these symptoms sound familiar to you, discuss them with your health care provider. 

How Common Is PMOS and Why Does It Go Undiagnosed?

PMOS affects roughly 1 in 10 women, making it one of the most common hormonal conditions of reproductive age.1,2 It's typically diagnosed using the Rotterdam criteria which states that a patient must meet two of the following three features to receive a PMOS diagnosis (provided that other conditions that mimic these symptoms have been ruled out):5

1.      Irregular menstrual cycles (less than 21 days or more than 35 days in length)

2.      Hyperandrogenism

3.      Polycystic ovaries on ultrasound

Despite how common PMOS is, and despite having a clear, internationally recognized diagnostic framework, it remains highly underdiagnosed. Symptoms like irregular periods or acne are often chalked up to stress, diet, or "just how my body is," which means many women live with PMOS for years before getting an answer.

Is PMOS Connected to Insulin Resistance?

Yes! Insulin resistance is one of the metabolic features that the new name “PMOS” was specifically designed to highlight. Insulin resistance happens when the body's cells stop responding efficiently to insulin, the hormone responsible for moving sugar out of the bloodstream and into cells for energy. To compensate, the body produces more insulin, and those elevated insulin levels can, in turn, push androgen production even higher.6

This creates a feedback loop that's central to understanding PMOS: elevated androgens disrupt ovulation, while insulin resistance can intensify androgen production, which can make symptoms harder to manage on the reproductive side alone.6 This is part of why the renamed condition leads with "polyendocrine" and "metabolic" rather than just "ovarian" as it points clinicians and patients toward looking at blood sugar and hormone regulation together, not in isolation.

How Is PMOS Diagnosed?

If you suspect PMOS, here's generally what the diagnostic process looks like:

  1. Symptom and cycle history. Your provider will ask about period regularity, hair growth, skin changes, and any fertility concerns.
  2. Bloodwork. Hormone panels can check androgen levels along with markers related to insulin and blood sugar regulation.
  3. Pelvic ultrasound. This looks at ovarian structure, though ovarian appearance alone isn't enough for a diagnosis.
  4. Ruling out look-alike conditions. Thyroid disorders and other hormonal imbalances can mimic PMOS symptoms, so providers typically rule these out first.
  5. Applying the Rotterdam criteria. Your provider weighs your symptoms and test results against this standardized framework to confirm a diagnosis.

Because many patients with PMOS struggle with their weight, they are unfortunately the victims of fat-phobia and weight bias when they start to discuss their concerns with providers. They are often told to “just lose weight” or “eat less, move more” which can feel incredibly frustrating when this advice simply does not work anymore despite their best efforts.

How Is PMOS Managed?

There's no single fix for PMOS, but a thoughtful, ongoing plan can make a real difference in symptoms and long-term health. Management generally includes:
